What are the responsibilities and job description for the HUD Service Coordinator position at Diakon?
- Responsible for helping residents obtain supportive services from the community that are needed to enable independent living and aging in place.
- Refers and links the residents to supportive services provided by the Community such as, case management, personal assistance, homemaker, meals-on-wheels, transportation, counseling, occasional visiting nurse, preventive health screening/wellness, and legal advocacy.
- Provides case management when service providers are not available through the general community.
- Creates a directory of providers for use by both development staff and residents.
