Description
The Social Worker provides patient support and education before and during dialysis. Working as needed , this position advocates for patients with appropriate local, state and federal resources and social agencies; attends quality improvement and patient care plan meetings and maintains case documentation.
Qualifications
MSW required with current social work license. At least 12 months of social work experience with dialysis patients preferred, have reliable transportation, and able to travel to local area sites.
