Description
The Home RN Intake Coordinator reviews medical records when presented for clinical determinations on appropriateness for our facilities and home patients. The position requires fast turnaround, attention to detail, excellent organizational skills, and clear, informative and supportive communication.
Responsibilities :
- Review of medical records when presented for clinical determinations based on Intake guidelines and clinical admission criteria;
- Collaborate and communicate with regional clinical leadership, clinical dens staff, SNF representatives, hospital case workers and on occasion, potential home patients and family members to support, inform and facilitate patients into our services;
- Review, edit, condense and track patient information in referral tracking systems census programs;
- Support Intake coordinators with referral processing based on volume and demand
- Perform other duties such as assigned projects, reviews and audits as requested or directed.
