We are seeking an experienced and motivated Clinical Nurse Manager to lead and support our growing care team in delivering Chronic Care Management (CCM), Remote Patient Monitoring (RPM), and other preventative care programs. This nurse will be responsible for supervising our clinical team, overseeing patient care workflows, managing a panel of patients, and driving performance through key clinical metrics. The ideal candidate is someone who thrives in a fast-paced, tech-enabled healthcare setting and is passionate about patient outcomes and team development.

Key Responsibilities:
  • Team Leadership & Oversight
o Supervise and support a team of Medical Assistants delivering remote proactive care services.
o Conduct regular team check-ins, provide clinical guidance, and facilitate skill development.
o Review care plans, documentation, and patient notes for quality and compliance.
  • Patient Care Management
o Manage a personal caseload of patients, providing monthly touchpoints and care coordination.
o Conduct clinical assessments, develop care plans, and escalate concerns as needed.
o Serve as a clinical liaison for patients with complex needs or transitions of care.
  • Program Oversight
o Ensure adherence to clinical protocols, documentation standards, and CMS guidelines.
o Collaborate with operations and support staff to streamline workflows and improve efficiency.
o Lead clinical quality assurance and documentation audits.
  • Escalation & Issue Management
o Triage and respond to clinical escalations from MAs or RPM monitoring alerts.
o Communicate with providers and patients to resolve clinical concerns promptly.
o Implement interventions and coordinate follow-up care.
  • Data & KPI Management
o Track and report on program KPIs such as enrollment retention, patient engagement, and outcome measures.
o Use data to guide coaching, identify training needs, and support strategic planning.
o Work cross-functionally with leadership to present findings and improvements.
