The Role :
The Hospice Clinical Manager is responsible for managing hospice care operations by overseeing patient case reviews coordinating interdisciplinary teams and providing clinicians with guidance to ensure quality patientcentered care. This role involves making home visits evaluating patient progress and adjusting care plans as needed while handling complex cases with compassion and efficiency. It requires a registered nurse license prior experience in hospice or home care and the ability to navigate sensitive situations collaborating closely with a dedicated team of clinicians social workers and medical staff to deliver holistic individualized care.

Key Responsibilities :
- Oversee hospice care operations using hospice expertise to manage and review patient cases ensuring care meets high standards of endoflife services.
- Provide leadership and guidance by mentoring clinicians to promote effective performance and compassionate care delivery. Coordinate closely with the interdisciplinary team and provide realtime support as needed.
- Plan and direct care services through home visits conferences and record reviews to assess patient needs and adjust care plans accordingly.
- Support and educate staff by setting immediate and longterm therapeutic goals and offering guidance on hospice care best practices.
- Collaborate with medical and operational teams to coordinate smooth transitions and maintain continuity of care for referred patients.Qualifications Registered nurse with current licensure to practice
