The Role :
The Occupational Therapist (OT) Home Health role involves delivering high-quality, in-home occupational therapy services to patients in East Portland and Gresham. The OT will conduct thorough assessments, create personalized treatment plans, and work closely with patients to help them regain or adapt skills to improve their independence and quality of life. This position is a Pay Per Visit role with an average earning potential of $93,000$98,000 per year, offering flexibility, patient-centered care, and collaboration with interdisciplinary team members.
Key Responsibilities :
- Conduct Comprehensive Assessments : Perform detailed assessments of each patient's functional status, including physical, psychological, and social aspects, to develop a holistic view of care needs.
- Develop Treatment Plans : Create individualized treatment plans and set measurable goals that support patient's unique needs and abilities, adjusting as necessary to facilitate progress.
- Educate and Empower Patients : Guide patients in regaining lost skills or adapting to new ways of completing tasks, offering practical strategies and tools for improving their daily functions.
- Collaborate with Interdisciplinary Teams : Work alongside other healthcare professionals to ensure a coordinated, holistic approach to patient care.
