Overview
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong background in patient care, with experience in urgent care settings preferred. As an LPN, you will play a vital role in providing high-quality nursing care to patients, assisting with assessments, and ensuring adherence to infection control protocols. Your ability to work collaboratively within a multidisciplinary team will be essential in delivering comprehensive care.

Responsibilities

• Conduct patient assessments and monitor vital signs to ensure optimal health outcomes.
• Administer medications and treatments as prescribed by physicians.
• Assist in urgent care situations, including ventilating patients when necessary.
• Perform laboratory duties such as phlebotomy and specimen collection.
• Engage in case management activities to coordinate patient care effectively.
• Provide education and support to patients and their families regarding health management.
• Implement infection control measures to maintain a safe environment for patients.
• Offer specialized care for infants, ensuring their unique needs are met.
• Lift and assist patients as required, ensuring their comfort and safety.

Requirements

• Submission Requirements
• 1 year Home Health experience - REQUIRED
• Travel experience - REQUIRED
• Active and clear Driver's license and Car Insurance - REQUIRED
• Hawaii license - REQUIRED
• BLS
• Homecare Home Base and OASIS - REQUIRED
• COVID and FLU vaccination - REQUIRED - will not accept declinations
• Arrhythmia certification - Preferred
