What You Will Do as a Hospice Network Liaison

As a Hospice Network Liaison, you will facilitate admissions from assigned hospitals, clinics, and MD offices into Stonerise agencies. You will match service locations’ capabilities to patient needs and facilitate patient flow from acute care providers into Stonerise to meet/exceed census and goals. Some of your primary responsibilities include:
- Communicating with the Interdisciplinary team daily to ascertain information and knowledge about open beds, scheduled discharges, scheduled admissions, hospitalized patients, clinical capabilities, and center events
- Managing the electronic referral system for assigned centers and develop professional relationships with case managers and discharge planners, hospital employees and management, patients, and families
- Completing clinical reviews via Allscripts and/or onsite evaluation as needed or requested, and coordinating a seamless transition to assigned centers
- Remaining visible as assigned hospital and being available as a resource to promote Stonerise in a positive manner
- Abiding to all HIPPA laws and observing confidentiality policy
- Presenting the benefits of long and short-term skilled nursing at Stonerise to patients, families, hospital employees, etc.

Education, Licensure & Certification Requirements
1-3 years of translatable experience in Hospice sales.
Current and valid driver’s license with available transportation and ability to drive
Previous work as a Hospice Liaison preferred.
