Position Summary
We are seeking a compassionate and experienced Nurse Practitioner (NP) to join our Home Health & Hospice team. The ideal candidate will work collaboratively with physicians, nurses, social workers, and other care providers to deliver comprehensive, high-quality care to patients in their homes.
Key Responsibilities
Conduct comprehensive assessments and develop individualized care plans
Provide in-home clinical evaluations
Collaborate with physicians and interdisciplinary team members for patient management
Prescribe medications and therapies in accordance with state regulations
Monitor and document patient progress and response to treatments
Participate in care team meetings and quality improvement initiatives
Ensure compliance with regulatory and organizational policies
Qualifications
Licensed Nurse Practitioner (NP) in [State]
Preferred 2 years of clinical experience (Home Health, Palliative, or Hospice experience preferred)
DEA license and prescriptive authority
Strong clinical judgment and decision-making skills
Excellent communication, organizational, and interpersonal skills
Ability to work independently in the field
