Job Summary
We are seeking a dedicated and compassionate Case Manager to join our team at Catholic Charities, Diocese of Venice in Florida. The ideal candidate will have experience working with youth, families, or special populations, and possess strong communication and relationship-building skills.
Key Responsibilities
- Crisis Intervention and Support: Provide crisis intervention, advocacy, referral, and coordination for the youth homeless demonstration rapid rehousing project.
- Client Support: Offer emotional support to clients throughout the case management process, helping them feel safe and understood.
- Service Coordination: Coordinate services with other agencies as needed to ensure clients' needs are met.
- Goal Setting: Help clients obtain needed services such as housing, medical care, education support, or vocational training, and develop long-term plans that address their needs in both the short term and long term.
Requirements
- Education: Bachelor's degree in a related field.
- Experience: 1 year of experience working with children, youth, families, or a special population, or an equivalent combination of education, experience, and training.
- Skills: Strong communication and relationship-building skills, ability to work with others in a warm, non-judgmental manner while being a role model, working knowledge of database reporting systems and Microsoft Office.
- Licenses/Certifications: Florida Driver's License, ability to pass a Level II background screening.
