Direct Access is seeking an individual to assist a primary care medical practice in the following areas :

  • Assist patients with Annual Wellness Assessments and documenting their Advanced Care Planning needs.
  • Screen patients for chronic & behavioral health related care management programs.
  • Assist with connecting patients to counseling and addiction support programs.
  • Assist with connecting patients to community resources and specialty disease education.
