- Assist patients with Annual Wellness Assessments and documenting their Advanced Care Planning needs.
- Screen patients for chronic & behavioral health related care management programs.
- Assist with connecting patients to counseling and addiction support programs.
- Assist with connecting patients to community resources and specialty disease education.

Qualifications
