Nurse Practitioner (Primary Care) - Amsterdam, NY
Job Type: Part-Time (Onsite)
Shift: 3 Shifts per week (5 hours per day)
Role Overview: We are seeking a dedicated and skilled Nurse Practitioner (NP) to deliver comprehensive primary care services to residents in assisted living facilities. This role offers a unique opportunity to work closely with an interdisciplinary team to enhance the quality of life for older adults in long-term care settings.
Key Responsibilities:
- Conduct health assessments including physical exams, medical history, and medication reconciliation.
- Develop and implement individualized care plans with the healthcare team.
- Manage chronic conditions and acute illnesses, providing necessary interventions and referrals.
- Order and interpret diagnostic tests to aid in diagnosis and treatment planning.
- Prescribe medications within the NP scope, following state regulations and facility policies.
- Participate in chronic care management services to improve health outcomes.
- Provide health education and counseling on disease prevention and self-care.
- Maintain accurate documentation in eClinicalWorks (eCW) electronic health records
- Engage in quality improvement initiatives and interdisciplinary meetings to enhance care standards.
Requirements:
- Master's degree in Nursing and successful completion of an accredited Nurse Practitioner (NP) program.
- Unrestricted New York State Nurse Practitioner license in good standing.
- Active DEA permit to prescribe medications.
- Eligible to provide reimbursable services and maintain provider status with Medicare and Medicaid.
- Minimum of 2-3 years of Adult/Geriatrics experience as a Nurse Practitioner.
- 3600 hours of clinical experience are required.
- Primary care experience is preferred.
