Position Summary:
This is a professional position providing Care Coordination to youth and families in Treatment Works 2.0 (TW 2.0) under the Outpatient Services Division. The TW 2.0 program provides children's therapy and High-Fidelity Wraparound (HFW) services that are designed for children who have experienced trauma, including loss and grief, depression, anxiety, abuse histories and addictions. These services can be provided in office or in the community. TW 2.0 services can be for the youth or the family, and assists in managing mild to moderate behavioral or emotional needs which can stem from trauma, including school behavior difficulties, adoption related issues, thoughts of suicide or self-harm, abuse histories, and addictions. All TW 2.0 programming utilizes the High Fidelity Wraparound model and a solution-focused, family centered approach and evidenced based or research supported practices to assist children and families in problem solving skills, behavior strategies, and anger management skills, among other therapeutic interventions.
This is a field position. The Care Coordinator is responsible for meeting an annual client facing client care number of productivity hours. Achieving the minimum required weekly, monthly and annual client facing client care hours is an essential function of this position.

EDUCATION EXPERIENCE/REQUIREMENTS:
- Bachelor's degree in Human Services from an accredited college with a minimum of one year of previous experience working with adults and children with serious persistent mental illness.
- Completion of Mental Health Care Coordinator Certification required.
- Must have excellent writing, communication, computer, organizational and time management skills with the ability to work independently and as part of a team.
- Proficiency in completing progress notes, treatment plans, and pertinent information concerning the consumer required.
