Position Summary:
This is a professional position providing Care Coordination to youth and families in Treatment Works 2.0 (TW 2.0) under the Outpatient Services Division. The TW 2.0 program provides an intensive wraparound approach to engaging and working with youth who have been identified as at risk for suicide. The goal of services is to ensure that youth and families are stabilized and prepared to remain in or reenter their educational environment. Services will be provided in the home, school, and community by a counselor and behavioral health case manager who will be working intensively to address the behaviors that resulted in the referral to the Treatment Works program.
This is a field position. The Care Coordinator is responsible for meeting an annual client facing client care number of productivity hours. Achieving the minimum required weekly, monthly and annual client facing client care hours is an essential function of this position.

EDUCATION EXPERIENCE/REQUIREMENTS:
- Bachelor's degree in Human Services from an accredited college with a Minimum of two years documented clinical experience in working with at-risk children and families.
- Completion of Mental Health Care Coordinator Certification required.
- Must have excellent writing, communication, computer, organizational and time management skills with the ability to work independently and as part of a team.
- Proficiency in completing progress notes, treatment plans, and pertinent information concerning the consumer required.
- Experience with school aged children with histories of suicidial thoughts or actions.
