POSITION SUMMARY:
This is a professional position providing job coaching to Directions for Living child services case managers who are certified, or identified to be certified, in Targeted Case Management (TCM). This position will work with Children's Outpatient (COP) under the Outpatient Services Division. COP provides children's therapy, TCM, and High-Fidelity Wraparound (HFW) services that are designed for children who have experienced trauma, including loss and grief, depression, anxiety, abuse histories and addictions. These services are provided both in office and in the community. COP services can be for the individual or the family, and assists in managing mild to moderate behavioral or emotional needs which can stem from trauma, including school behavior difficulties, adoption related issues, thoughts of suicide or self-harm, abuse histories, and addictions. All COP programming utilizes a solution-focused, family centered approach and evidenced based or research supported practices to assist children and families in problem solving skills, behavior strategies, and anger management skills, among other therapeutic interventions.
The Child Services Job Coach will assess children's services TCM's strengths and areas in need of improvement to ensure TCM's are meeting requirements of their position and to ensure that child services TCM's are providing high quality care. The Child Services Job Coach will then be responsible developing a training plan and providing one-on-one training tailored to the needs of children services TCM team members for the purpose of improving and maintaining job skills related to TCM services. The Child Services Job Coach will be responsible for in field training and shadowing of child services TCM's to ensure high quality TCM field and home visits. The Child Services Job Coach will be responsible for instructing child services TCM's on skills related to assessing risk and identifying the appropriate level of care for the client being served. (TCM, Wraparound, Higher Level of Care, etc.). The Child Services Job Coach will be responsible for monitoring DFL Behavioral Health Case Management Certifications and ensure all new staff submit certifications timely. The Child Services Job Coach will be responsible for understanding and training child services TCM's on how to accurately and effectively complete Targeted Case Management Authorizations.
EDUCATION EXPERIENCE/REQUIREMENTS:
- Master's Degree in human services, or other related field preferred, minimum of Bachelors degree required.
- Certified as a Behavioral Health Case Manager/Supervisor through the Florida Certification Board
- Certification in CPI Nonviolent Crisis Intervention techniques and First Aid / CPR required.
- Obtain and maintain the High Fidelity Wraparound Facilitator and Coaches Certification.
- Certified Professional in Healthcare Quality preferred.
- Healthcare experience with quality assurance background - preferred.
- Experience in AHCA and CMS regulatory requirements - preferred.
- Experience in quality and risk preferred.
- Knowledge of DCF Policies and procedures.
- Knowledge of CARF standards.
- Knowledge of information systems and software applications.
- Strong customer service skills.
- Ability to build relationships with both internal and external stakeholders.
- Ability to use EHR.
- Must have excellent interpersonal and communication skills
- Strong problem solving and time management skills with the ability to work independently and as part of a team.
- Ability to stay organized when dealing with multiple employees.
