POSITION SUMMARY:
This is a professional position providing facilitation of the Integrated Decision Team (IDT) for the Mobile Medical Unit (MMU) as part of Health Care for the Homeless (HCH) through the Pinellas County Department of health and in partnership with Direction for Living's (DFL) Housing First Division. The MMU provides primary care to homeless individuals with DFL providing psychiatric, counseling, and case management services, both in the field as part of the Mobile Medical Unit as well as at Bayside Clinic.
The Integrated Decision Team Facilitator- Mobile Medical Unit (IDT Facilitator) is a position providing Wraparound staffing services, utilizing natural supports, community supports, and input and resources from other community partners or professional agencies. The IDT Facilitator works together as part of a team with a counselor and case manager. The IDT Facilitator is responsible for ensuring program staffing's follow the evidence-based practice of Wraparound in addition to ensuring that safety and risk issues involving the client are identified and mitigated. The IDT Facilitator ensures the client has an individualized treatment plan that is strength based and built collaboratively with each member of the team during the IDT session. The treatment plan, risk level, and clinical interventions are developed by the Integrated Decision Team that consists of the client, natural supports, and formal supports that care about and know the client best. Each IDT member, including the client, contributes valuable knowledge, expertise and perspective that informs the treatment plan and clinical and case management response. The IDT Facilitator ensures the client is an integral part of the team and must have ownership of the plan. The IDT Facilitator ensures no planning sessions occur without the presence of the client. Clients are engaged as equal partners in creating systems for behavior change. The IDT Facilitator ensures that services are Culturally & Linguistically Competent and are tailored to the unique culture of the client. The IDT Facilitator will ensure that IDT's occur in the community at a place convenient and comfortable for the client. This is a field-based position. The IDT Facilitator is responsible for meeting an annual client facing client care number of 1,410 hours. Achieving the minimum required weekly, monthly and annual client facing client care hours is an essential function of this position.

EDUCATION EXPERIENCE/REQUIREMENTS:
- Must hold at least a bachelor's degree Social Work or related area of study from an accredited university; or a master's degree in social work or related area of study. With a minimum of one year Case Manager experience.
- Must become certified as a Wraparound Facilitator in high fidelity Wraparound within 3 months of employment. Preference will be given to individuals certified as a Wraparound Facilitator in high fidelity Wraparound.
