Essential Functions
The Direct Support Professional (DSP) will implement transition support services for youth and young adults ages 12-30 with developmental disabilities eligible This cross-trained position serves individuals through both the Transitions Program and the Community Habilitation Program to foster independence and achieve valued outcomes. Services will be provided in Allegany, Cattaraugus, and Chautauqua Counties.
- Deliver transition support services in collaboration with the Transition Support Lead.
- Assist individuals in becoming more independent in activities of daily living and achieving goals outlined in Life Plans.
Principal Activities
Transitions Program Activities:
- Meet Family Support Services (FSS) grant goals, serving a minimum of 50 individuals annually with 3,000 service hours.
- Collaborate with local schools and community organizations to identify and support youth benefiting from person-centered transition planning.
- Conduct outreach to promote program services in the community.
- Maintain compliance with documentation and submit records in a timely manner.
- Refer individuals to community organizations and support systems to create a robust circle of support.
- Support the team in accomplishing program-related goals.
Community Habilitation Activities:
- Provide support for individuals to gain community or home-based experiences aligned with their goals.
- Ensure effective instruction and safety by adhering to Staff Action Plans.
- Facilitate volunteer and recreational opportunities that enhance independence.
- Ensure documentation is complete, compliant, and submitted on time.
- Attend and complete all required training.
Job Qualifications
- Education: Degree in Human Services or Education preferred; High School Diploma or GED required.
- Experience: Minimum of 2 years working with individuals with developmental disabilities, special education, or in a human services environment.
- Strong desire and compassion to work with diverse populations.
- Proficiency in Microsoft Office and documentation practices.
- A valid driver’s license with a clean driving record and current auto insurance.
- Excellent verbal and written communication skills.
- Strong organizational and time management skills, with the ability to manage multiple tasks effectively.
- Ability to interpret policies, procedures, and written materials.
- Must be able to comply with HIPAA requirements.
- Must exhibit respect for colleagues, staff, and program participants.
