What are the responsibilities and job description for the Program Manager - No Wrong Door position at Disability Network of Mid-Michigan?
Essential Functions:
Conducts or oversees the following independent living services: Person-Centered Options Counseling for No Wrong Door (PCOC-NWD) and State Health Insurance Partnership Program (SHIP)/ Medicare Improvements for Patients and Providers Act (MIPPA) services.
· Supervises staff, including receiving and assigning referrals, scheduling work hours, evaluating performance and service delivery, resolving conflict, conducting team meetings, and ensuring that staff receive required training and certifications
· Interview/hire/train new staff as required
· Attend initial and refresher training courses as required for the No Wrong Door program.
· Conducts presentations and outreach as requested about services.
· Participate in check-ins, planning, and update meetings with grant partners.
· Acts as back-up for team members during vacation, absence, or low staffing hours
· Prepares written reports and billings as required by funding source and submits monthly data to appropriate staff on a timely basis.
· Ensures the accurate and timely documentation of team.
· Provide team availability on at least a monthly basis to statewide referral service for scheduling.
· Ensures that services and activities are implemented with strict adherence to the DNMM Code of Ethics, governmental and contractual requirements and the Independent Living Philosophy.
· Maintains consumer data and financial records as required by contracting agencies, grant funders, and the law.
· Attends DNMM meetings and community meetings as assigned.
· Participates on local, state and/or national committees for the purpose of promoting the Independent Living Philosophy
· Monitor staff hours, approve PTO, review all data entry and approve timesheets, overtime, etc.
· Provides on-call services as assigned.
· Follow agency policies and procedures.
· Promote and encourage independence for all people with disabilities.

Competencies/Position Requirements:
-Knowledge of HIPAA compliance
-Familiarity with Mandated Reporter guidelines
-Knowledge and application of Person-Centered Processes
-Knowledge of and experience with developmental disabilities and Mental Health disabilities
-Familiarity with scheduling/tracking of due dates

Quality and quantity standards:
Documentation within 24 hours, completion of above standards by month end each month, work with individuals in the environment of their choice, follow Mandated Reporter Guidelines, managing multiple projects at one time, consistently meet deadlines, providing effective presentations when required and effectively manage a hybrid team including remote employees and employees stationed in different offices and counties throughout the service area.
Education:
Bachelor’s degree in health & human service-related field, equivalent life experience, or CHW certification preferred.
Expected hours of work:
Salaried position based on a 40-hour workweek, with willingness to work additional hours when needed. Flexibility in scheduling additional work time outside of regular business hours may occasionally be necessary, especially in cases where community events are taking place and DNMM presence is beneficial or required.
Physical factors:
- Travel to and from community locations within a 12-county radius is expected.
- Entering individual homes and/or community locations where steps may be present
- Carrying/moving items, climbing stairs
- Travel to and from community locations
- Prepare for and set up tables/pop-up tents and materials for community events.
