Independent Living Coordinator Position
JOB DESCRIPTION:
To assist DRRC participants, either individually and in groups, in maintaining or increasing their level of independence in the community. To provide peer support, advocacy and independent living skills training to consumers in an effort to build the self-esteem, confidence and competence needed to achieve independence.
QUALIFICATIONS:
The Independent Living Coordinator may be person with a disability who has achieved or maintained a substantial level of independence and can share their life experience freely and constructively with consumers. Minimum high school degree or equivalent. The Independent Living Coordinator should have an awareness of disability issues, good communication skills and self-motivation.
ESSENTIAL JOB FUNCTIONS:
· Counsel consumers individually regarding personal problems, issues, and concerns related to their disability, independence and emotional growth. Be a positive role model.
· Schedule and organize a twice-monthly group-counseling meeting for DRRC consumers.
· Assist each consumer in establishing an Independent Living Plan (ILP) based on needs, goals, and disability adjustments.
· Maintain confidentiality of consumers.
· Maintain contact with consumers on a regular basis.
· Provide complete, concise information regarding consumer’s progress in his/her file.
· Complete monthly and quarterly reports in a timely manner.
· Function as a team member with other staff, coordinating schedules, intakes, and reporting.
· Maintain contact with supervisor. Keep supervisor informed of important issues, events, concerns, and needs of participants.
· Maintain quality documentation, records and reporting of activities including units of service in the agency's data collection system.
· Participate in staff meetings and program meetings.
· Assist in planning DRRC and execution of DRRC activities as assigned.
· Represent DRRC at community functions as assigned.
· Promote positive community awareness in regard to disability issues.
· Participate in staff training functions.
· Additional duties as assigned by supervisor.
