Essential Task
- Will be responsible the day-to-day activities of the assigned program and review services.
- Monitor the services provided for assigned client case load.
- Completes required documentation/billing accurately and timely.
- Assure that all program and licensure standards are met according the DBMD guidelines.
- Maintain positive relationships and open communication with clients, client families, case managers, vendors and other concerned parties.
- Attend and participate in all agency manager training sessions and other training approved by the company.
- Must be able to work independently and manage time effectively.
- Addresses attendant disciplinary action when necessary.
- Will perform client care and on-call duties as assigned or needed.
- Reviews and corrects any outstanding issues in reference to Quality Improvement Program.
- Travel required for client visits and community activities as necessary.
- Observes confidentiality at all times.
- Complies with federal, state and company policies, procedures and regulations.
- Performs other duties as assigned.
- A bachelor’s degree in a health and human services related field and a minimum of two years of experience in the delivery of direct services to individuals with disabilities; or
- Have an associate’s degree in a health and human services related field and a minimum of four years of experience providing direct services to individuals with disabilities; or
- Have a high school diploma or certificate recognized by a state as the equivalent of a high school diploma and a minimum of six years of experience providing direct services to individuals with disabilities;
- and either: Is fluent in the communication methods used by an individual to whom the case manager is assigned (for example American sign language, tactile symbols, communication boards, pictures, and gestures); or
- Within six months after being assigned to an individual, becomes fluent in the communication methods used by the individual.
- Must be able to demonstrate competence with computer applications such as MS Word and Excel and have the ability to readily learn agency specific software.
- Proficiency in the use of database and spreadsheet software.
- Must have reliable transportation, a valid Texas driver’s license and auto liability insurance.
- Must not be convicted of a crime, offense or misdemeanor as defined in TAC
- 558.241(b), relating to Management and not barred from employment by state law, including Nurse Aide Registry and Employee Misconduct Registry.
- Must have and maintain current CPR certification including the Heimlich Maneuver, First Aid, online certification is not sufficient to meet this requirement.
- Completion of agency’s DBMD training and HHSC Computer Based training.
- Must be able to type 35 wpm with 80% accuracy and 80% accuracy in Excel.
