Responsibilities:
- Responsible for the customer service and management of private duty services and aides.
- Develops and implements community outreach strategies that promote partnerships within the community and industry.
- Establishes mechanisms for measuring quality care and organizational performance and participates in agency performance improvement projects.
- Intake client calls, provide client communication and follow up.
- Propose services and financial agreements with clients.
- Accurately input new patient referral and contact information in to the system and coordinate with schedulers and RN immediately for scheduling and assessments.
- Reviews documentation and assessments submitted and creates and maintains care plans
- Maintains compliant and up to date client records
- Maintains an ongoing relationship with current, future and former clients, client families, company employees and any outside agencies.
- Assists in the recruitment, interviewing, selection, and orientation of the Aide team members.
- Participating in orientation of new hires, including skills check of and supervision of care.
- Provides required supervision of caregivers and review of care plans, routinely
- Participates in on call rotation
Qualifications:
- RN license is required
- MUST have Medicare and Private Duty Experience
- Preferred experience with Wellsky and/or Kinnser
- Minimum of 2 years home health care experience required
- Minimum of 2 years management/supervisory experience in private care and/or home health preferred
- Ability to analyze and interpret financial metrics and key performance indicators
- Strong interpersonal and relationship building skills
- Strong leadership and communication skills
- Must comprehend the basics of personal care, housekeeping and meal preparation
- Must understand and respect client’s including ethics and confidentiality of care.
- CPR certification is required
