Position Summary:
We are currently seeking a dedicated and compassionate MSW (Master of Social Work) to join our Home Health team on a PRN (as needed) basis in Naples, Florida. The MSW will be responsible for assessing the psychosocial needs of our home health patients and providing support and resources to address those needs. This is a rewarding opportunity to make a positive impact on the lives of patients and their families in the comfort of their own homes.
Key Responsibilities:
- Conduct comprehensive psychosocial assessments of patients to identify their social, emotional, and financial needs
- Develop individualized care plans in collaboration with the interdisciplinary team to address the identified needs
- Provide counseling and support to patients and their families to help them cope with illness, disability, or end-of-life issues
- Advocate for patients to access community resources and support services
- Educate patients and families about advance care planning, mental health concerns, and other relevant topics
- Maintain accurate and up-to-date documentation of all patient interactions and care provided
Qualifications:
- Master's degree in Social Work from an accredited university
- Current Florida license as a Licensed Clinical Social Worker (LCSW)
- Previous experience working in a home health setting is preferred
- Strong communication and interpersonal skills
- Compassionate and empathetic attitude towards patients and their families
- Ability to work independently and as part of a team
- Proficiency in computer skills and electronic medical record documentation
