What are the responsibilities and job description for the Mental Health Case Manager position at District 19 Community Services Board?
District 19 CSB offers a challenging full time Mental Health Case Manager position to provide case management to children and adolescents located in Petersburg, VA. Essential job duties of this position include; assessment and planning services to include developing an ISP per Medicaid regulations; monthly contacts, activities or communication relevant to the ISP; Coordinating services and treatment planning with other agencies and providers; psychotropic medications; providing crisis and behavioral interventions.
Mental Health Case Management means- services that can include assistance to individuals and their family members in accessing needed services that are responsive to the person's individual needs. Case management services consists of: identifying potential users of the service; assessing needs and planning services; linking the individual to services and supports; assisting the individual directly to locate, develop, or obtain needed services and resources; coordinating services with other providers; enhancing community integration; making collateral contacts; monitoring service delivery; discharge planning; and advocating for individuals in response to their changing needs.
Mental Health Case managers are hired with the goal of becoming welcoming, recovery-oriented, and co-occurring competent to engage all individuals receiving services in empathetic, hopeful, integrated relationships to help them address multiple issues successfully.
Successful applicant must have: Bachelor’s degree in Human Services; Must meet QMHP qualifications per Medicaid regulations; Proficiency with personal computers and at least one year of clinical experience with children and adolescents.
Employees or contractors providing case management services shall have knowledge of:
1. Services and systems available in the community including primary health care, support services, eligibility criteria and intake processes and generic community resources;
2. The nature of serious mental illness, intellectual disability, substance use disorders, or co-occurring disorders depending on the individuals served, including clinical and developmental issues;
3. Different types of assessments, including functional assessment, and their uses in service planning;
4. Treatment modalities and intervention techniques, such as behavior management, independent living skills training, supportive counseling, family education, crisis intervention, discharge planning, and service coordination;
5. Types of mental health, developmental, and substance abuse programs available in the locality;
6. The service planning process and major components of a service plan;
7. The use of medications in the care or treatment of the population served; and
8. All applicable federal and state laws and regulations and local ordinances.
Employees or contractors providing case management services shall have skills in:
1. Identifying and documenting an individual's need for resources, services, and other supports;
2. Using information from assessments, evaluations, observation, and interviews to develop service plans;
3. Identifying and documenting how resources, services, and natural supports such as family can be utilized to promote achievement of an individual's personal habilitative or rehabilitative and life goals; and
4. Coordinating the provision of services by diverse public and private providers.
Employees or contractors providing case management services shall have abilities to:
1. Work as team members, maintaining effective inter- and intra-agency working relationships;
2. Work independently performing position duties under general supervision; and
3. Engage in and sustain ongoing relationships with individuals receiving services.
Must meet the DBHDS definition of "Qualified Mental Health Professional-Child (QMHP-C)"
To qualify as a QMHP-C, the individual must have the designated clinical experience and must either (i) be a doctor of medicine or osteopathy licensed in Virginia; (ii) have a master's degree in psychology from an accredited college or university with at least one year of clinical experience with children and adolescents; (iii) have a social work bachelor's or master's degree from an accredited college or university with at least one year of documented clinical experience with children or adolescents; (iv) be a registered nurse with at least one year of clinical experience with children and adolescents; (v) have at least a bachelor's degree in a human services field or in special education from an accredited college with at least one year of clinical experience with children and adolescents, or (vi) be a licensed mental health professional.

This is a S-N1. Salary range $43,261.00 - $48,667.00

District 19 Community Service Board is a qualifying employer for public service loan forgiveness (PSLF).
