Job Title: Case Manager

Job Description:
Job Purpose:
Establish a person-centered process for coordinating the supports, range of services, instruction and assistance needed by persons with developmental disorders.
Job Duties:
· Assist consumer and/or their legal representatives with re-determination of financial eligibility as required by the DHHR office in the county where the consumer lives.
· Verify financial eligibility during monthly home visits.
· Participate in discharge process and provide linkage to services appropriate to the level of need when a member is found to be ineligible for I/DD Waiver Services during annual eligibility review.
· Assist with procurement of all services that are appropriate and necessary for each member within and beyond the scope of the I/DD Waiver program.
· Provide education, linkage, and referral to community resources.
· Monitor to ensure that the member’s health and safety needs are addressed.
· Meet with assigned consumers at least monthly and with their supports at the consumer’s
· residence to verify that services are being delivered in a safe environment, in accordance with the
· IPP and that services are being appropriately documented.
· Meet with assigned consumers in non-residential settings, including facility-based day program
· supported employment sites and volunteer locations.
· Comply with all company/state/federal reporting requirements.
· Coordinate necessary evaluations to be utilized as a basis of need and recommendation for
· services in the development of the IPP.
· Support consumers as necessary to convene and conduct IDT meetings and notify IDT member
· 30 days in advance of meeting.
· Promote a valuable and meaningful social role for assigned consumers in the community while
· recognizing their unique cultural and personal value system.
· Act as advocate for assigned consumers.
· Provide oral and written information about the I/DD Waiver provider agency rights and
· grievance procedures for consumers.
· Communicate with other service providers on the IDT to allow for continuity of services.
· Coordinate and develop consumer Individual Program Plans (IPP) annually and review at least
· every 90 days unless specified in the plan.
· Coordinate critical juncture and discharge/transfer meetings and services.
· Access the necessary resources detailed in the IPP, make referrals to qualified services providers
and resources, and monitor that service providers implement the instruction, behavioral and
service objectives of the IPP.
· Interface with required agencies on behalf of assigned consumers regarding the assessment process, and the purchase of services/budget development.
· Disseminate copies of IPP to the IDT members within 14 days.
· Provide planning and coordination before, during and after crisis.
· Provide information and assistance regarding consumer-directed service options during annual IPP.
· Present proposed restrictive measures to the Human Rights Committee.
