Hospice Liaison

Your responsibilities include:
Develop strategic marketing plans: to grow referrals and census.
Build relationships: with healthcare professionals, physicians, and community partners.
Educate referral sources, patients, and families: on hospice care benefits and services.
Represent our hospice: at community events and healthcare facilities.
Conduct initial meetings: with patients and families, explaining hospice services and obtaining consent.
Track and analyze marketing activities: to assess effectiveness and find areas for improvement, in collaboration with the Lead Liaison.
Desired qualifications:
· Healthcare marketing or sales experience.
· Strong communication and interpersonal skills.

· Knowledge of hospice and end-of-life care.

Social Work Responsibilities:
Patient and Family Support:
Provide comprehensive psychosocial assessments, counseling, and support to patients and families facing end-of-life care, addressing their emotional, social, and spiritual needs.
Resource Navigation:
Facilitate connections for patients and families to necessary community resources, including financial assistance, grief support, and other pertinent services.
Care Planning:
Collaborate with the interdisciplinary team to develop and implement individualized care plans that cater to each patient's unique requirements.
Bereavement Support:
Offer continuous bereavement support to families following the loss of a loved one.
Documentation:
Ensure accurate and thorough documentation of patient assessments, interventions, and progress.
Marketing Responsibilities:
Community Outreach:
Develop and execute marketing strategies aimed at increasing awareness of hospice services within the Sun City community.
Relationship Building:
Cultivate and maintain strong relationships with healthcare professionals, referral sources, and community organizations.
Information Dissemination:
Provide detailed and comprehensive information about hospice services to potential clients and referral sources.
Promotional Materials:
Create and distribute professional marketing materials, including brochures, website content, and social media posts.
Data Analysis:
Monitor and analyze marketing efforts to measure their effectiveness and identify opportunities for improvement, in collaboration with our Lead Liaison.
Qualifications:
· MSW or equivalent.
· Hospice, palliative care experience.
· Strong communication, interpersonal, and organizational skills.
· Independent and teamwork abilities.
· Compassionate patient care approach.
· Knowledge of home health and hospice regulations.
· Healthcare sales/marketing experience (preferred).
· Bilingual skills (preferred).
· EMR proficiency.
