The Social Service Director is responsible for planning, organizing, developing, and directing the overall operation of the Social Services Department. This must be done in accordance with current federal, state, and local standards, guidelines, and regulations, our established policies, and procedures, and as directed by the Administrator. The Social Services Director interacts with the residents, families, visitors, other facility staff, and community agencies during the admission and discharge process. The Director interacts with residents, families, visitors, and other facility staff. This position is responsible for patient assessments, care planning, and supervising additional social services staff. Assists in the development of policies regarding participation in facility planning for health and welfare services. Provides input to strategic decisions as related to social services areas of responsibility. Proactively plans and facilitates de-scalation protocol with the support of the interdisciplinary team and can resolve escalated situations within the facility.
- Facilitates and Coordinates the Admission Process and Discharge Planning
- Directs the Overall Operations of the Social Services Department
- Communicates and Interacts with Other Staff, Families, Residents. Community Agencies and Visitors
- Maintains Healthy Relationships and Uses Professional Conduct with Residents, Families, Visitors, Supervisors and Co-workers
- An Ohio license to practice social work
