What are the responsibilities and job description for the Human Services Coordinator II: Acquired Brain Injury (ABI)/Moving Forward Plan (MFP) position at DMR-Western Region?
The Department of Developmental Services (DDS) is filling a challenging Human Services Coordinator II (HSC II) position in the Central/West Regional Office and Clark St Satellite Office. This position supervises the Acquired Brain Injury program known as the ABI/MFP (Acquired Brain Injury / Moving Forward Plan) Program waiver pursuant to the Hutchinson settlement agreement. The HSC II will function as the Service Coordinator Supervisor for the ABI/MFP program and will be responsible for providing direct and adequate supervision to the other Service Coordinators who comprise the Regional ABI/MFP team. They will support and monitor the HSC performance and ensure compliance with statewide guidelines.
The Human Services Coordinator Supervisor will work closely with other members of the team to offer guidance, initiate activities to enhance the performance of the Department, offer ongoing consultation to individual members of the team and may carry a small caseload of ABI/MFP participants, which will require travel to meetings within the Region. The HSC II is directly supervised by the Director of ABI/MFP Waiver Central/West.
Preferred Qualifications: Working knowledge of HCSIS and Meditech databases.
Please Note:
This position is located in the Regional Office located in both Springfield, MA and Worcester, MA
Schedule: 9:00am - 5:00pm
Days Off: Sunday/Saturday
The incumbent of this position will be required to have some flexibility in their schedule as there may be occasional night and weekend meetings with individuals, guardians, families and other agencies. Driving will be part of this assignment. This position will oversee Service Coordinators in Springfield and Worcester locations.
Driver's License is required. Travel throughout the Central/West Region and some statewide travel.
