Occupational Therapist (OT) – Home Health
Position Overview:
We are seeking a dedicated and compassionate Occupational Therapist (OT) to provide skilled occupational therapy services to patients in their homes. This role is essential in helping patients regain independence and improve their daily living skills in accordance with physician orders and a personalized plan of care. Under the supervision of the Director of Rehabilitation Services, the OT plays a vital role in patient recovery, caregiver education, and interdisciplinary collaboration.
This position involves:
- Developing and evaluating patient-centered treatment plans in partnership with the patient, their representative, and caregiver(s).
- Administering therapeutic interventions to enhance functional abilities and independence.
- Educating patients, families, and caregivers on adaptive techniques and rehabilitation strategies for daily activities.
- Communicating and collaborating with physicians and healthcare providers to ensure continuity of care.
- Supervising Occupational Therapy Assistants (OTAs) and Home Health Aides (HHAs) as assigned.
- Contributing to Quality Assurance and Performance Improvement (QAPI) initiatives and participating in interdisciplinary assessments.
Qualifications & Requirements
- Licensure: Active Occupational Therapist (OT) license in the state of practice
- Experience: Minimum of two (2) years of experience (Home Health preferred)
- Certifications: CPR certification (American Heart Association or American Red Cross)
- Skills & Abilities:
- Strong clinical judgment with the ability to work independently in a home health setting
- Excellent verbal and written communication skills for patient and caregiver education
- Commitment to delivering high-quality, patient-centered care
- Proficiency in electronic health records (EHR) documentation
- Additional Requirements:
- Valid driver’s license, auto liability insurance, and reliable transportation
- 100% travel required within the service area
Education Requirements
- Graduate of an accredited Occupational Therapy program, recognized by:
- Committee on Allied Health Education and Accreditation of the American Medical Association
- American Occupational Therapy Association (AOTA)
- National Certification eligibility through the American Occupational Therapy Association
- High School Diploma or GED required
Work Environment & Physical Requirements
- Provides care in patient homes and travels within the coverage area
- Requires physical activity such as kneeling, squatting, reaching, twisting, climbing, and lifting (up to 100 lbs with assistance)
- Must be able to tolerate varied home environments, bodily fluids, and hazardous materials
- Must meet all state-mandated health requirements, including an annual TB test
Schedule & Compensation
- Schedule: Monday – Friday (8:30 AM – 5:00 PM)
- Compensation: Salary or Pay Per Visit (PPV)
