Summary of Essential Functions for the Position:
Comitted to Caring, Physical Therapy Assistant (LPTA), we are seeking a dedicated and compassionate Licensed Physical Therapy Assistant (LPTA) to provide intermittent physical therapy services in patients' homes. Under the direction and supervision of a Registered Physical Therapist (PT), the LPTA will administer treatments in accordance with physician orders and individualized care plans. The PT will oversee and evaluate the LPTA’s performance in treatment administration, patient education, data collection, and communication regarding patient progress and health needs.

Qualifications Licensure & Certification:
- Active Physical Therapy Assistant (PTA) license in the state(s) of practice.
- CPR certification from the American Heart Association or American Red Cross.
- Minimum of two (2) years of experience, with home health experience preferred.
- Strong initiative and ability to exercise independent judgment.
- Excellent oral and written communication skills.
- Strong interpersonal and customer service skills with a commitment to providing high-quality patient care.
- Demonstrates alignment with the core values and mission of the organization.
- High School Diploma or GED.
- Graduate from an accredited two-year Physical Therapy Assistant program approved by the American Physical Therapy Association.
- Valid driver’s license, reliable transportation, and auto liability insurance.
- Ability to travel 100% of the time as required.
- Compliance with state-mandated health screenings, including annual TB tests and other required medical examinations.
- Must be able to tolerate exposure to various environmental conditions, including temperature fluctuations, odors, bodily fluids, and hazardous materials.
- Work is performed in patient homes with frequent travel between locations.
- Requires significant physical activity, including kneeling, squatting, reaching, twisting, climbing, walking, and assisting in lifting/transferring patients up to 100 lbs.
- Must possess adequate hearing and vision abilities or utilize adaptive devices to perform essential job duties.
- Shift: Monday – Friday, 8:30 AM – 5:00 PM
- Coverage Area: Cleveland, TX
