Job Title: Speech Language Pathologist (SLP) – Home Health
Position Summary:
As a Speech Language Pathologist (SLP) in Home Health, you will provide high-quality speech therapy services to patients in their homes, ensuring effective care in accordance with physician orders and a personalized plan of care. Under the supervision of the Director of Clinical Services/Clinical Manager, you will develop, implement, and evaluate individualized treatment plans in partnership with the patient, caregivers, and other healthcare professionals. Your role also involves patient, caregiver, and family counseling as well as education to promote optimal recovery and quality of life.
Key Responsibilities:
- Provide speech therapy services to patients in their homes, per the plan of care.
- Develop and evaluate individualized care plans in collaboration with the patient and their family.
- Provide counseling and education to patients and caregivers regarding speech and language disorders.
- Collaborate with physicians and other healthcare providers to ensure continuity of care.
- Contribute to the Quality Assurance and Performance Improvement (QAPI) program and participate in interdisciplinary assessments.
- Ensure that speech therapy services are delivered in compliance with regulations and standards.
Qualifications & Requirements:
- Licensure: Active Speech Language Pathologist (SLP) license in the state of practice.
- Experience: Minimum of two (2) years of experience in Home Health preferred.
- Certifications: CPR certification (American Heart Association or American Red Cross).
- Skills & Abilities:
- Strong clinical judgment and ability to work independently in a home health setting.
- Excellent oral and written communication skills.
- Ability to build and maintain strong interpersonal relationships with patients, caregivers, and healthcare teams.
- Flexibility and initiative to adapt in a dynamic work environment.
- Additional Requirements:
- Valid driver’s license, auto liability insurance, and reliable transportation.
- 100% travel required within the service area.
Educational Requirements:
- High School Diploma or GED required.
- Master’s Degree in Speech-Language Pathology from an accredited program.
- Certification of Clinical Competency in Speech Pathology or Audiology from the American Speech-Language-Hearing Association (ASHA), or in the process of accumulating supervised experience required for certification.
Physical Requirements & Working Conditions:
- Work will be conducted in patient homes and office settings.
- Requires physical effort, including kneeling, squatting, reaching, twisting, climbing, walking, and assisting with the lifting and/or transferring of patients up to 100 lbs.
- Must be able to tolerate exposure to varied home environments, bodily fluids, and hazardous materials.
- Must meet state-mandated health requirements, including an annual TB test.
