Job Title: Registered Nurse Case Manager (RN), Home Health
Job Summary: The Registered Nurse Case Manager (RN) is responsible for delivering skilled nursing care to patients in their homes on an intermittent basis, following physician orders and an established plan of care. This role operates under the direction and supervision of the Director of Clinical Services and involves patient, caregiver, and family counseling and education. The RN Case Manager coordinates care with all physicians and healthcare providers involved in the patient’s plan of care, ensuring comprehensive, quality healthcare services. Additionally, the RN Case Manager participates in ongoing interdisciplinary assessments, quality assurance and performance improvement (QAPI) programs, and supervises Licensed Vocational Nurses (LVNs) and Home Health Aides (HHAs).
Essential Responsibilities:
- Administer skilled nursing care to patients in accordance with physician orders and the established plan of care.
- Educate patients, caregivers, and families on disease management, medications, and overall care needs.
- Communicate effectively with physicians and other healthcare providers to ensure seamless coordination of care.
- Participate in the development, review, and revision of individualized patient care plans in collaboration with the patient and their caregivers.
- Contribute to quality assurance and performance improvement (QAPI) initiatives.
- Supervise and guide Licensed Vocational Nurses (LVNs) and Home Health Aides (HHAs) in patient care.
- Maintain accurate and timely documentation of patient conditions, care provided, and response to treatment.
- Be available for on-call duty at least once per month.
- Travel to patient homes and provide services in varying environments.
Qualifications:
- Active Registered Nurse (RN) license in the state of practice.
- Minimum of two (2) years of nursing experience; home health experience preferred.
- Current CPR certification from the American Heart Association or American Red Cross.
- Strong clinical judgment, initiative, and problem-solving skills.
- Excellent oral and written communication skills with strong interpersonal abilities.
- Commitment to providing quality patient care and adherence to agency core values and goals.
- Valid driver’s license, auto liability insurance, and reliable transportation.
- Willingness to travel as needed (100% travel may be required).
Education Requirements:
- High school diploma or GED required.
- Graduate of an accredited school of professional nursing.
Working Conditions & Physical Requirements:
- Work is conducted in patient homes and the agency office, requiring frequent travel.
- Physical demands include standing, kneeling, squatting, reaching, twisting, climbing, walking, and assisting with lifting and transferring patients up to 100 pounds.
- Must have functional vision and hearing abilities or appropriate adaptive devices.
- Required to complete an annual TB test and other state-mandated health screenings.
- Must be comfortable working in varying environmental conditions, including exposure to odors, bodily fluids, and hazardous materials.
Work Schedule:
- Monday to Friday, 8:30 AM – 5:00 PM
