Job Summary:
We are seeking a dedicated and experienced Nurse Practitioner to join our primary care team. The ideal candidate will provide patient-centered care, focusing on health promotion, disease prevention, and the management of acute and chronic conditions.
Key Responsibilities:
- Patient Care: Conduct routine check-ups, physical exams, and age-appropriate screenings for conditions such as diabetes, hypertension, and high cholesterol.
- Diagnosis and Treatment: Address acute illnesses (e.g., colds, flu, infections) and minor injuries. Prescribe medications and recommend treatment plans.
- Chronic Disease Management: Help manage chronic conditions such as diabetes, high blood pressure, asthma, and heart disease. Monitor symptoms and adjust treatment plans as needed.
- Preventive Care: Administer vaccinations and immunizations. Provide lifestyle counseling to promote better health outcomes.
- Mental Health Support: Screen for anxiety, depression, and other mental health conditions. Provide counseling and medication management when necessary.
- Coordination of Care: Refer patients to specialists for advanced treatment and collaborate with other healthcare professionals to ensure continuity of care.
- Patient Education: Educate patients and their families about health conditions, treatment plans, and preventive measures.
Qualifications:
- Licensure: Current and valid Nurse Practitioner license in the state of Florida.
- Education: Master’s degree in Nursing or a related field from an accredited program.
