- Work collaboratively with clients, their families, and support systems to identify needs and develop individualized service plans.
- Conduct thorough assessments of client situations, determine the most effective response, and ensure timely intervention in crisis situations.
- Collaborate with schools, courts, healthcare providers, law enforcement, and other community resources to achieve positive outcomes for clients.
- Advise clients on available services and provide guidance on accessing necessary support.
- Maintain accurate and detailed records of all assigned cases, including written assessments, reports, and other documentation as required.

- Bachelor's degree in Social Work or a related field, with eligibility for the Social Work national exam within specified timeframes.
- Bachelor's degree in a related Human Services program from an accredited university, with eligibility to enroll in SWTC-approved courses to secure State of Wisconsin Social Work Training certificate within established timeframes.
- Master's degree in Social Work and current Social Work Certification within the State of Wisconsin.
- Knowledge of laws and regulations affecting social service delivery.
- Familiarity with community resources and alternative care options.
- Working knowledge of mental health and AODA assessment, diagnosis, and treatment.
- Ability to maintain confidentiality and handle sensitive information effectively.
