Under the direction of the Home Health Manager plans and directs home care services and is experienced in social services with emphasis on community health education and resources. The professional Medical Social Worker builds from the resources of the community to plan and direct services to meet the needs of individual and families/caregivers within their homes and communities. The social worker acts as a facilitator coordinator advocate educator and resource for clients and families. Participates in the overall home health plan of care based on a psychosocial assessment client history physician orders expected outcomes and client goals. Initiates appropriate interventions to facilitate patients' ability to participate in the plan of care and remove barriers to reaching goals. Coordinates with the patient/family the interdisciplinary team physician and community agencies to provide for long-term care planning or placement assistance referrals to community resources short term counseling and support services and end of life concerns/ advance directives. Provides home health staff consultation and support.
