SUMMARY / OBJECTIVE
Assist and support individuals to maintain overall health, develop skills in activities of daily living, identify, access, and engage in their community and home environments, access public resources, facilitate relationship building with the greater community, and to empower persons in services to achieve their goals and the life they want.

PREFERRED EDUCATION and EXPERIENCE
- High School Diploma or GED
- Valid driver's license, reliable transportation
- 1 year of experience working with individuals with disabilities
- Ability to engage with others using courtesy, tact, and good judgment.

- Appropriate safety equipment and first aid kit in personal vehicle.

TRAVEL / EXPECTED HOURS OF WORK
Travel will be required. The employee is expected to have reliable and safe means of transportation to transport clients to appointments, community engagement activities, and other activities in the community.
Expected hours of work varies based upon program and case assignment and can include early mornings, daytime hours, evenings, weekends, and holidays.
