Job Summary

Duties
- Provides case management, care coordination, and discharge planning for inpatient and outpatient settings.
- Assists service members, veterans, and families in accessing appropriate healthcare services for complex conditions.
- Coordinates care across multiple providers, addressing medical, nursing, and psychosocial needs.
- Assesses patients for case management through referrals, evaluations, and direct contact.
- Develops and implements patient-specific care plans with short- and long-term goals.
- Guides patients and families through the healthcare system, ensuring access to necessary services.
- Monitors and updates treatment plans per military and healthcare guidelines.
- Evaluates patient adherence, treatment outcomes, and resource utilization.
- May require travel to inspect military living quarters and advocate for patient needs.

Experience:
- full time clinical case management experience with children, families, seniors, or groups.
- Minimum of 2 years for BSN full-time experience in nursing and care coordination.
- Must be knowledgeable in medical privacy and confidentiality (Health Insurance Portability and Accountability Act [HIPAA]
Education:
- Graduate from baccalaureate degree (BSN) program in nursing.
Certification:
- Have and maintain current certification in Basic Cardiac Life Support (BLCS) by the American Heart Association and certification in Advanced Cardiac Life Support (ACLS).
- Certified Case Manager by Commission for Case Management (CCM) or American Nurses Credentialing Center (ANCC) -OR- certification eligible with 2 years’ experience for BSN.
