Description

Conducts health education and policy advocacy in assigned program areas around chronic diseases, injuries, tobacco control, and behavioral health with community partners to support policy change and other population-based prevention activities.

Examples of Duties

ESSENTIAL DUTIES AND RESPONSIBILITIES : (The following examples are illustrative only and are not intended to be all inclusive.)

  • Participates in community coalitions, partnerships, and collaborations to support and advance work, including youth coalitions and youth-adult partnerships.
  • Develops and fosters partnerships with a variety of partners and stakeholders.
  • Provides education and training to key stakeholders on selected health topics. Participates in community assessment processes.
  • Works with key stakeholders and community-based partners to advocate for health-related policies on the local level.
  • Provides education, resources, and related support to city staff and local elected officials.
  • Supports community-based and governmental organizations such as schools, housing authorities, workplaces, parks and recreation departments and districts, non-profits, and others in development of organizational policies that promote health.
  • Uses assessment data and evidence-based practice guidelines to make recommendations for strategies to improve the public health policy landscape, considering the varied needs of specific populations, such as issues related to age, gender, culture, and health disparities.
  • Supports community education efforts including mass media campaigns, social media, community outreach events.
  • Works with other team members to ensure consistent messaging in communities and across the agency.
  • Provides education, both internally and externally, to raise awareness and understanding of chronic diseases, tobacco, injuries, and behavioral health.
  • Participates in implementation of DCHD's Strategic Plan as it relates to chronic disease, tobacco prevention, substance abuse, and behavioral health.
  • Participates in program development, grant writing, and grant implementation including program work plans and budgets. Participates in the timely completion of applicable grant progress reports and other documentation as required.
  • Collaborates with program evaluators in data collection and other evaluation related activities.
  • Participates in relevant grantee meetings and trainings as required by funders.
  • May participate on department-wide cross-divisional work groups and projects.
  • Represents the Department on community prevention committees and other relevant groups as assigned.
  • Other duties as assigned.

SUPERVISION RECEIVED : General supervision is provided by the Assistant Director for Community Health or Executive Director.

SUPERVISORY RESPONSIBILITIES : None.

INDEPENDENT JUDGMENT : This position requires a moderate level of independent judgment and understanding. Work is performed in accordance with program manuals, policies, procedures, and regulations. Incumbent has ability to problem solve and recommend solutions to management when issues arise outside of their scope.

Minimum Qualifications

EDUCATION and / or EXPERIENCE :

Requires a bachelor's degree from an accredited college or university with major coursework in public health, health promotion, health communication, social work, education, or a related field. Master's Preferred. Requires a minimum of two years of related work experience.

An equivalent combination of related education, training, and experience that demonstrates the knowledge, skills, and ability to effectively perform the functions of this position may be considered.

KNOWLEDGE, SKILLS AND ABILITIES :

Knowledge :

  • Practical implementation of adult and youth educational methods
  • Principles and practices of youth engagement, coalition building, and developing community partnerships.
  • Principles and practices of population-based public health addressing chronic diseases tobacco control, injuries, and behavioral health.
  • Shared risk and protective factors for youth prevention work.
  • Trauma-informed care and adverse childhood experiences (ACEs).
  • Social determinants of health.
  • Skills :

  • Excellent written and verbal communication skills.
  • Engage youth, public and private organizations, and members of the community.
  • Facilitate and lead community and committee meetings.
  • Skilled in various software packages and Microsoft Office, including Word, Excel, PowerPoint, and Outlook.
  • Abilities :

  • Establish and maintain effective working relationships with other employees, representatives of other agencies and organizations, and members of the community.
  • Maintain sensitive and confidential client information.
  • Work effectively with individuals and groups in diverse multi-cultural and socio-economic environments.
  • CERTIFICATIONS, LICENSES, & REGISTRATIONS :

  • Bilingual (English / Spanish) skills preferred.
  • This position requires successful completion of a criminal background check including fingerprinting through a national database.
  • Must possess a valid Colorado Driver's License upon hire, with complying insurance. Review of motor vehicle record is required at time of hire and periodically throughout employment.
