Description
The CST Coordinator operates under WI Statute 46.56 in the provision of the Coordinated Services Team (CST) program. CST provides wraparound services to children currently engaged in services identified in the statute including, but not limited to mental health intervention, child protection, and juvenile justice.
Full-time based on 37.5 hours/week, 1950 hours/year.
Essential Duties

- Receive and screen potential applicants to the CST program.
- Coordinate and facilitate the CST wraparound meetings.
- Assist families with developing their wraparound team including both their informal and formal support(s).
- Perform the strengths and needs assessment (CANS Assessment).
- Complete required data collection as outlined in state/program standards.
- Develop, implement and review the plan of care with the identified CST team according to timelines.
- Meet with families in their homes and other locations of their choosing as needed (at minimum on monthly basis).
- Documentation and distribution of CST meeting minutes to all stakeholders on the team within 2 business days.
- Weekly communication with any HHS unit sharing a case with a CST client.
- Facilitate the CST Coordinating Committee quarterly.
- Maintain client records.
- Attend all required CST training and meetings.
- Complete the state documentation for annual and 6-month review, and performance report for the CST program.
- Perform other duties as assigned.
Minimum Qualifications
Must possess and maintain a valid driver's license. At minimum an Associate’s Degree in Human Services or combination of training and experience equivalent to four years. Experience facilitating meetings required. Bachelor's Degree in a Human Services Field is preferred.
- Demonstrated work experience in building and maintaining positive relationships with diverse stakeholders, fostering a collaborative environment to achieve common goals.
- Demonstrated work experience coordinating complex schedules, arranging logistics, preparing comprehensive agendas and minutes, and facilitation to ensure meetings run smoothly and efficiently.

Working Conditions
This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. This role works frequently in varying environments including private residences, schools, or medical facilities, and may utilize a personal vehicle to transport clients.
Physical Requirements
The CST Coordinator will be required to perform non-sedentary and sedentary work. Non-sedentary work includes walking distances on uneven surfaces and climbing stairs; standing and sitting at variable times; carrying a lap top computer and other materials simultaneously; and ability to lift up to 30 pounds. May require field visits, sometimes in buildings or terrains that are not handicapped accessible. Requirements include driving a vehicle to and from appointments. Must maintain a valid driver's license.
