Summary

  • JOB SUMMARY
    • This is a grant funded role expected through August 2028. The primary role of the RN Care Facilitator will be to provide care coordination services to all patients enrolled in the Plus One Doula Program during the perinatal period and up to one-year postpartum. The RN will work across the inpatient and outpatient care continuum to ensure that patient care is enhanced across the Maternal Child Health service line. Identifies OB patients needing extra services and enrollment in the Plus One doula program. Functions as a facilitator of inter-disciplinary collaboration across care transitions. Coaches target patient populations and their caregivers to assume an active role in the formation and execution of a plan of care, encourage self-management, and assist in coordination of communication between the patient and primary care providers. Assesses patient and family understanding of pregnancy, high-risk factors, and continually educates patient through perinatal period and up to one year postpartum. Collaborates and communicates with care providers both internal and external to coordinate service delivery and achieve optimal patient outcome of care. Collaborates and communicates with hospital care management teams to identify postpartum needs and coordinate a postpartum plan of care. Collaborates and communicates with community partners such as but not limited to public health nursing, mental health providers, postpartum and breastfeeding support groups, inpatient and outpatient medication assisted treatment programs, Early Childhood Family Education, and Headstart programs.
  • MINIMUM QUALIFICATIONS
    • Education: See Licensure/Certification/Registration requirements.
    • Experience: One (1) year nursing experience. Demonstrated knowledge and skill in computer system and data collection.
    • Licensure/Certification/Registration: Current license in Minnesota to practice as a Registered Nurse. BLS Certification with a designation of either BLS Provider or Healthcare Provider from either the American Heart Association, American Red Cross or Military Training Network.
  • PREFERRED QUALIFICATIONS
    • Education: Bachelor’s Degree In Nursing or Master’s degree in health related field such as public health, nursing, hospital administration, healthcare analytics, population health.
    • Experience: Three (3) years nursing experience. Background in Maternal Child Health, quality improvement work and data interpretation.

  • KNOWLEDGE, SKILLS AND ABILITIES
    • Knowledge of current practice in the nursing care of perinatal patients, newborns, and families. Excellent written and oral communication skills. Well-developed level of comfort working with physicians and care providers. Ability to build consultative relationship to work collaboratively and to develop successful partnerships with others. Experience in creative program developing and marketing. Ability to work successfully with medical staff, volunteers and community representatives. Ability to exercise good judgement and accept personal responsibility, establish priorities, and to maintain patient confidentiality.
    • READING - Intermediate: Ability to read and interpret documents such as operating and maintenance instructions and procedure manuals.
    • WRITING - Intermediate: Ability to write routine reports, correspondence, or procedures.
    • SPEAKING - Intermediate: Ability to effectively present information in one-on-one, small group situations or before groups of customers, clients, and other employees of the organization.
    • MATHEMATICAL SKILLS - Basic Skills: Ability to add and subtract two-digit numbers and to multiply and divide with 10's and 100's. Ability to perform these operations using units of American money and weight measurement, volume, and distance.
    • REASONING ABILITY - Intermediate Skills: Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
    • AGE SPECIFIC COMPETENCIES: Skilled at assessment and knowledgeable of growth and development. Provides appropriate care respective to the ages of the patients served per clinical environment.
