We are seeking a qualified person to be part of our interdisciplinary team that provides social service case management to short-term and long-term residents.
Working as a resident and family social service assistant, you will be responsible for:
- advocating for resident rights
- evaluating and meeting resident psycho-social needs
- arranging for services such as hospice
- developing a comprehensive assessment and resident-based care plans
- exercising independent judgment, making sound decisions, and exhibiting flexibility when needed
- ensuring and providing therapeutic interventions to assist residents in coping with their transition and adjustment to their placement
- serving as liaison between residents, families, the facility, and outside agencies
- providing oversight for our long-term care residents psycho/social needs
Required Qualifications
- bachelor's degree in social work or in a human services field including but not limited to: gerontology, sociology, rehabilitation, counseling, psychology.
- excellent communication and listening skills
- ability to organize and prioritize effectively
- good problem solving and assertiveness skills
