THE POSITION
The Village East Social Services is responsible for getting to know each resident personally, ensuring their social, emotional, and psychological needs are met. Meeting with each residents medical team and loved ones to discuss recent successes or setbacks to their goals, developing treatment plans. Must be highly compassionate and possess strong customer service skills.
THE HOURS: 8am to 5pm Monday - Friday
THE RESPONSIBILITIES
- Develop a social history, social assessment and care plan which identifies pertinent problems and needs, realistic goals to be accomplished, and the specific action to be taken in resolution of the problems and/or needs upon admission of each new resident.
- Participate in the pre-admission screening of all potential residents and provide information to residents and families during admission.
- Must be adaptable, flexible to changes, and able to prioritize and organize work efficiently to accomplish workload within time allotted.
THE QUALIFICATIONS
- Graduation from high school with at least three years’ experience in some capacity working with older adults, with at least one year of the experience being in a long-term care facility.
- College course work in sociology, psychology, gerontology, or social work may be substituted for up to two years' experience.
- Valid social work license preferred, not required.
- Must be able to relate professionally, positively, and cooperatively with residents, resident's families and employees.
- Must possess proficient computer skills, including email use.
- Must be familiar with: residents' rights and responsibilities; regulations applicable to long term care providers; and facility's policies and procedures.
- Must demonstrate strong communications skills and be able to work comfortably and effectively with residents (at various cognitive and functional levels), family members, and co-workers.
- Professional in appearance and conduct.
- Must be criminally cleared.
- Be in good health and physically and mentally capable of performing assigned tasks.
- Good physical health
