We are looking for a SASH Coordinator to join our team!
Downstreet is seeking an enthusiastic and dedicated SASH Coordinator to help Downstreet residents age safely at home, meet basic needs, achieve personal stability, and thrive. This person will provide direct services and connections to community resources for resident in Warren, Waitsfield, Waterbury, Montpelier, Barre, and Cabot.
Key Responsibilities
- Identify needs of individuals and develop healthy living plans to meet those needs. Develops and maintains a regular Community Health Living Plan
- Connect participants to community resources and services, and follow-up on engagement and outcomes
- Coordinate opportunities for participants to develop social connections and build community
- Thoroughly explain the Use and Disclosure Agreement and related documents to participants and their family members/supports
- Adheres to confidentiality guidelines and appropriate documents information releases
- Establish connections and maintain strong working relationships with local service providers. Represent Downstreet and SASH at relevant community meetings
- Maintain collaborative relationships with participants, Downstreet staff, and community partners. Including the ability to work individually and as part of a team
- Complete all administrative duties related to the position, including participant files, program monitoring, and reporting in a timely manner
- Ensure compliance with all relevant legal, regulatory, and organizational requirements
Experience and Education
- Bachelor’s degree and 3 years of experience in social work, or an equivalent combination of education, employment, and life experience
- Previous experience coordinating services specifically for older adults and/or people with disabilities is preferred
- Proficient in Microsoft Office Suite and ability to learn relevant property management software
- Highly organized with a strong ability to manage priorities and deadlines and ability to manage confidential information
- Strong written and verbal communication skills, active listening skills, and problem-solving abilities
- Must have knowledge of and appreciation for individual autonomy and choices
- Commitment to equity and inclusion, and social and economic justice
- Knowledge of area resources and programs available to older adults and people with disabilities is strongly preferred
- Valid Vermont Driver’s License and reliable personal vehicle
Physical Requirements
- Prolonged periods sitting at a desk and working at a computer
- Must be able to lift 25 pounds at a time
- Ability to access all Downstreet sites, buildings, apartments, and facilities
- Must be adaptable to working in an ever changing, high pressure work environment
