Job Definition :
The SAGE Program Coordinator is a member of the multi-disciplinary team with the purpose of providing a spectrum of administrative and clinical support to the program. This position is supervised by the SAGE Senior Program Manager to ensure the program is complying with applicable laws, regulations, and contract requirements.
The SAGE Program Coordinator must be willing and able to work in a fast-paced environment where staff availability and program needs are often changing. This position will help cover any administrative duties including coverage needs, oversee daily operations when needed, and assist program management to help relieve administrative burden with hiring, resource procurement, and assisting with specialist projects. This position will assist in providing routine administrative tasks related to Medicaid Integrated Managed Care (IMC), Collective Medical Technology online platform, Washington Administrative Codes (WAC), Revised Code of Washington (RCWs), King County Integrated Care Network (KCICN), Behavioral Health and Recovery Division (BHRD) Provider Manual, HIPAA privacy rulings, and more. Considerable attention and skills are needed in data systems and tracking for compliance. Time will be spent coordinating with other support departments within DESC to help prepare for yearly audits to assure compliance with all local, state, and federal laws and relevant contract and procedure requirements associated with the operations within the clinical programs. Models and promote core DESC values and participate in and provide training for staff to ensure the safety of programs.
MAJOR DUTIES AND RESPONSIBILITIES :
- Work in coordination with QIM department, OPBH Coordinator, Process Improvement Manager, Clinical Health Information Management Data Analyst, Training Manager and Specialist, Business Planning Manager, etc. to achieve the work needed for the SAGE program to achieve both cross program integration with housing / shelter, and medical teams, as well as, clinical programs.
- Work in coordination with Clinical leadership to provide oversight of program evaluation related to contract changes, workflow protocols, and service delivery. Identify additional support, resources, and information needed to support staff.
- Work with Clinical Program Coordinator, SAGE Managers, and Supervisors to develop training and onboarding program via Learning Center. Create training materials and tools to help staff successfully implement change and maintain quality.
- Provide support to SAGE leadership in coaching their teams on how to adopt clinical practice changes and how this applies to direct practice work.
- Collaborate with facility staff as needed for daily clinic operations as needed.
- Coordinate with CHASERS staff and clinical staff to implement needed changes for contract compliance to improve documentation or reporting systems for these programs.
- Represent DESC at external stakeholder meetings (as assigned) and communicate information back to the SAGE leadership team.
- In conjunction with SAGE leadership, assist with project start up or ongoing project improvement.
- Comply with all agency policies and procedures, Medicaid Integrated Managed Care (IMC), Collective Medical Technology online platform, Washington Administrative Codes (WAC), Revised Code of Washington (RCWs), King County Integrated Care Network (KCICN), Behavioral Health and Recovery Division (BHRD) Provider Manual, HIPAA privacy rulings, and more.
- Assist in various quality assurance activities, including but not limited to internal and county / state audits.
- Assist with quality improvement projects as assigned.
- Monitor service and other program data in order to track performance and compliance in identified areas; assess and report on efforts following prompts to SAGE clinical staff.
- Generate and update Bar Reports for review in site team meetings; generate monthly prompts for non-Medicaid applications and renewals;
- Support SAGE Senior Program Manager, Project Managers and Clinical supervisors with a wide range of activities including working with confidential medical & personnel information, files, and reports.
- Participate in managerial level meetings and provide support for hiring, termination, and investigation activities as directed.
- Assist in the entry and tracking of Facilities and Information Services work orders, coordinate with outside vendors as appropriate.
- Have experience in administering Federal funding requirements.
- Support clinical supervisors liaise with key partners and stakeholders, to ensure effective operations and communication with existing and new clinical programs.
- Provide administrative support to hiring teams and assist in training new employees.
- Coordinate a wide range of project logistics and resource needs across clinical programs which are in several different locations. Must be willing to travel to other locations.
- Attend and participate in relevant team meetings, agency sponsored trainings, and all-staff meetings.
- Other duties as assigned.

Support clients with achieving and maintaining healthy living conditions. This can include but is not limited to attending care conferences related to living conditions, outreaching and supporting clients in their residential units with tools and skills to maintain their units, coordinating with housing staff, participating in cleaning out clients' units, and documenting barriers to maintaining healthy living conditions.
