Job Description : PURPOSE OF JOB : The Cardiovascular Service Line Clinical Data Specialist draws upon a strong clinical nursing background and comprehension of the most current clinical guidelines for treatment of cardiovascular diseases representing multiple sub-specialties in the service line. The role functions as the liaison between professional cardiovascular societies and our cardiovascular physicians, extenders and care teams. Data abstraction begins the process and moves through analysis and aggregation of data. The data is formatted into presentations reporting activities reflecting cardiovascular patient outcomes using both internal and national benchmarks from American College of Cardiology, American Heart Association, Society of Thoracic Surgeons, Vascular Quality Initiatives and others as assigned. The specialist brings analysis of clinical care outcomes that identify variances related to abstracted topics; as well quality improvement opportunities / initiatives for cardiovascular patients. Prepares summaries of care during patient stay, as necessary to provide physician or extenders opportunities identified for resolution prior to discharge ensuring patients receive optimal evidence base care measures.ESSENTIAL FUNCTIONSResponsible for fully managing all aspects of assigned clinical data registries per contracts with American College of Cardiology, American Heart Association, Society of Thoracic Surgeons (or other entity) including abstraction, submission, analysis and resolution of data quality reports (35%)Conducts on going analysis of clinical outcomes for variances and missed opportunities toprovide evidence based clinical care, meet external benchmark standards and publicly reported outcomes (25%)Provide relevant patient care summaries as needed to consult with physicians and extenders for purposes of clarification of documentation and / or missing documentation as needed.Serve on multiple Quality Committees with analysis of potential root cause as related to trends and variations in care.(25%)Serves as data champion of Quality Committee Meetings and ongoing Accreditation / Certification planning meetings in preparation for program survey with summarized cases as needed. Provide relevant patient care summaries as needed to consult with physicians and extenders for purposes of clarification of documentation and / or missing documentation as needed.Serve on multiple Quality Committees with analysis of potential root cause as related to trends and variations in care.Work closely with clinical care teams to identify potential process improvement activities base on variances from registry findingsCollaborate with quality committee teams covering a wide range of clinical care monitoring across the continuum from pre-hospital care to discharge and follow for 30 daysMonitor 30 day readmission patterns for trends or insightsSafeguard patient care record privacy while navigating multiple sources - EMS platform, E-Clinical Works, Meditech (both Doylestown Hospital and Grandview Hospital), sending and receiving faxed information as needed with adherence to HIPPA guidelinesRemain current in new guidelines, clinical care statements, research studies at Doylestown Hospital that may impact current registry abstraction.Create feedback reports for clinical care providers with portrayal of care time line, highlights and outcomes for quality meetings, patient safety committees4. Serves as data champion of Quality Committee Meetings and ongoing Accreditation / Certification planning meetings in preparation for program survey with summarized cases for ongoing data needs related to program. (20%)Works closely with Quality Committee members as well as Clinical Program Accreditation Coordinator(s) for preparation and presentation for meetingsProvide clear and concise summary of potential clinical care outliers to accreditation coordinator and clinical care teams for real time resolutionCollaborate with accreditation coordinator(s), clinical care coordinators and leadership for data and analysis needsAttend and participate in specified quality committeewith in-depth knowledge of data elements, outcome measure inclusionexclusion criteriaPrepare chart analysis for presentation by the Medical Directors for educational purposes as requested related to changes in clinicalguidelinesServe as expert in interpretation of data specification and intent of registryCreate educational / power point or other documents helping care team understand new concepts and changes in evidence based care measures and documentationParticipate in annual clinical program re-accreditation cycles with Joint Commission and American College of Cardiology as data expert for assigned registryProvide lists for case inclusionsDrive computer to showcase examples of care provided as requested by auditor-Play a pivotal role in team creating or updating templates showcasing evidence of all care measures a patient qualifies to receiveProvide researched information as to the latest treatment of care for multiple subspecialties, ensuring essential patient education is clearly providedJob Qualifications : QUALIFICATIONS : Registered Nurse Required (college degree preferred)Recent substantial experience in direct care of cardiovascular patientsExperience with national registry data abstraction / analysis processesCare of patients in a hospital setting, minimum of 5 years' experience preferredFamiliarity with clinical program accreditation / certification processesDetail oriented, analytic ability to utilize available resources for decision makingAbility to work independently in a time oriented environmentProficiency with Meditech and ECW electronic medical recordsProficiency with Microsoft Suite for data presentation.Flexibility and ability to work in a dynamic, team oriented environment
