Job Description:
PURPOSE OF JOB:
Provides direct nursing care to Home Care patients. Is responsible for case management and coordination of other professional and para-professional services. This will be performed with respect to the appropriate specific chronological age, developmental age or social maturity of the patient.
II. ESSENTIAL FUNCTIONS:
- Promotes patient safety through adherence to policies, procedures, guidelines, Joint Commission standards, and state and Medicare regulations. This includes following physician orders. (20%)
- Demonstrates excellent clinical competency at all times according to departmental and program standards as well as the standards of each discipline, 100% supervisory visit compliance and completes competencies on time. (20%)
- Demonstrates patient case management by assessing, planning, implementing, and documenting patient care needs according to department guidelines in order to achieve quality outcomes. Documentation compliance is followed. (20%)
- Communicates and collaborates with patients, families, physicians, and others to provide coordination of care and customer service. IDT attendance is required on days clinician is working. (20%)
- Demonstrates fiscal responsibility by controlling costs, and enhancing revenue. Authorization obtained and all visits approved. (20%)
Job Qualifications:
QUALIFICATIONS:
- Education: Registered Professional Nurse with current license with Bureau of Professional & Occupational Affairs in the Commonwealth of Pennsylvania. BSN preferred.
- Experience: One year recent medical-surgical experience or community health experience desired. Two years of experience preferred.
- Other Skills: Current CPR certification. Current drivers' license and auto insurance. Ability to drive a car in inclement weather. Auto in good working condition. Professional attire for patient care. Basic Computer skills.
