Key Responsibilities:

  • Provision of comprehensive primary care services for all ages in a primary medical care clinic setting.
  • Provide care consistent with evidence-based medicine.
  • Perform minor outpatient procedures in accordance with the scope of practice and privileging policy.
  • Provide consultation to other clinic providers either in person or by telephone.
  • Consult with clinic physicians, specialists, or other health care professionals as appropriate.
  • Assess patient needs for specialty referrals, diagnostic and in-patient care and make appropriate referrals.
  • Provide patients and family members with counseling and education regarding health maintenance, disease prevention, diagnosis, treatment, and need for follow up as appropriate during each patient visit.
  • Verify and document that the patient understands diagnosis, treatment and follow-up recommendations as required by uniform data system, meaningful use and patient centered medical home standards.
  • Dispense medications in accordance with State of Mississippi Pharmacy Board policies.
  • Participate in a shared call system with other providers.
  • Assist with meeting clinic patient care team productivity and quality care standards.
  • Participate in development and utilization of practice guidelines and procedures.
  • Participate in audits, and meet standards of Quality Improvement, Risk Management and Utilization Management programs.
  • Complete accurate and timely documentation in patient records and encounters within 24-48 hours after the patient visit.
  • Maintenance of credentials essential for practice, including licensure, certification and CME.
  • Demonstrate respectful, professional and appropriate behavior that supports a team-oriented work environment.
  • Demonstrate a commitment to the mission, core values and goals of Mallory CHC and its healthcare delivery within our programs and services.
  • Participate in the health promotion/disease prevention activities required by the U.S. Public Health Services.
  • Attend all medical staff meetings and participate in the Community Health Center's Quality Assurance Program.
  • Perform other necessary duties as required by the Community Health Center to meet the goals of providing primary health care.

Qualifications:

Education: Graduate of an accredited Nurse Practitioner Program (i.e. Master's or Doctorate)

Experience: Minimum 1-3 years' experience in a medical clinical setting preferred

Certifications/Licenses:

  • Current Advanced Practice Registered Nurse (APRN) Licensure
  • Current Nurse Practitioner certification in specialty area
  • Current AANP or AANC certification
  • Current provider card in Basic Life Support (BLS)

Skills & Abilities:

  • Proficient in conducting comprehensive assessments, diagnosing, and developing treatment plans for patients of all ages.
  • Skilled in performing physical exams, screenings, and health assessments.
  • Knowledge of evidence-based practices for managing chronic and acute conditions.
  • Strong communication skills for educating patients and families about health conditions, treatment options, and preventative care.
  • Excellent active listening skills to understand patient concerns and deliver effective care.
  • Empathy and professionalism when working with patients of all ages and backgrounds.
  • Proficiency in electronic health record (EHR) systems or the ability to learn new software quickly.
  • Ability to work collaboratively with a multi-disciplinary team.
