Job Summary
We are seeking a Case Manager to join our team and oversee the coordination and management of patient care for our chronic care patients. This role will be responsible for assessing patient needs, developing personalized care plans, and coordinating services to ensure continuity of care across multiple healthcare settings. The ideal candidate will have a strong background in case management, care coordination, and a deep understanding of patient advocacy and support.
Key Responsibilities
Care Coordination & Patient Advocacy
Manage and coordinate the care of patients, ensuring timely and appropriate services are delivered across various healthcare providers.
Develop individualized care plans that address patients’ physical, emotional, and psychosocial needs.
Act as a liaison between patients, families, healthcare providers, and insurance companies.
Monitor patient progress and make adjustments to care plans as necessary.
Assessment & Evaluation
Conduct comprehensive patient assessments to identify care needs and barriers to successful treatment.
Regularly review and update care plans to ensure they align with evolving patient needs and clinical goals.
Assess patients' social determinants of health and work with community resources to support patients effectively.
Education & Support
Educate patients and their families on disease management, treatment options, and available healthcare services.
Provide resources and referrals to community services and programs that can assist with non-medical needs.
Offer emotional support to patients and families, ensuring they feel informed, supported, and empowered.
Compliance & Documentation
Ensure all case management activities are in compliance with state and federal regulations, as well as company policies.
Maintain accurate and up-to-date documentation of patient interactions, care plans, and progress notes in the electronic medical record (EMR) system.
Assist in audits and quality reviews as necessary.
Collaboration & Leadership
Collaborate with interdisciplinary healthcare teams, including physicians, nurses, and social workers, to provide comprehensive patient care.
Lead patient care meetings to ensure all team members are aligned on treatment goals.
Work closely with leadership to identify opportunities for program improvement and patient care enhancements.
Qualifications
Bachelor’s degree in Nursing, Social Work, or a related healthcare field (Master’s preferred).
Case management certification (CCM, ACM, or similar) preferred.
Minimum 3-5 years of experience in case management or patient care coordination.
Strong knowledge of healthcare services, care coordination, and patient advocacy.
Excellent communication, organizational, and problem-solving skills.
Proficiency in electronic medical records (EMR) systems.
