JOB TITLE: Case Manager / Intensive Case Management Specialist
REPORTS TO: Program Director, Adult Services
Position Status: Regular, part-time/full-time hourly non exempt employee from 24-36 hours/week
This position provides intensive case management, which includes street outreach, and care coordination for individuals who have been referred by police officers to the Law Enforcement Assisted Diversion (LEAD) project.

Education and Experience Requirements:
- High school diploma or equivalent
- Minimum of 1 year of related work experience
- Preferred: Bachelor's degree in social work, psychology, or related fields; or Community Health Worker or Peer Support Worker certification. Experience will be considered as a substitute for education.
- Preferred: Experience in case management and working with individuals with substance use disorder.
Primary Responsibilities:
- Communicate effectively with outreach staff, Police Department staff, and LEAD partners as needed regarding engaging participants
- Develop an individualized care based plan with client
- Monitor and update care plan through regular communication with Clinical Supervisor, PD and case conferencing with other providers
- Advocate with appropriate agencies for services and assist participants in the scheduling and coordination of services
- Conduct field and home visits as appropriate
- Accompany participants to appointments as appropriate
- Maintain professional relationships and effective communication with the participating Police Departments, LEAD partners, and service providers
- Assist with compiling and reporting data for monthly and quarterly reports to funding sources and LEAD evaluation team
- Attend LEAD Operational Workgroup Meeting and other scheduled meetings
- Participate in the on-call phone rotation schedule
- Maintain valid driver's license and clean driving record based on DreamTree Project insurance requirements
- Participate in other duties as assigned
Additional Compensation:
- $100 = 30 Days after start date
- $100 = 60 Days after start date
Job Type: Full-time
Pay: $20.00 - $25.00 per hour
