Part-Time/Full-Time Case Manager: Taos
Hours: Part-Time/Full-time, Hourly, Non-Exempt. Works 24-36 hours weekly.

All Staff Requirements:
- Provide service with integrity, respect, empathy, and confidentiality to youth and their families
- Maintain a safe and structured environment to provide service that is trauma-informed and is consistent with the principles of positive youth development
- Connect youth and family to needed resources, services, and activities while supporting youth and team-defined Personal Plan goals.
Education & Experience Requirements:
- Bachelor’s degree in social work, psychology, or related field preferred.
- Experience in case management and working with target population youth preferred.
Primary Responsibilities:
- Maintain a clear and complete TLP Case Manager folder.
- Review TLP applications and conduct interviews to recommend to the Program Director if a youth will be accepted into our Transitional Living Program.
- Meet with incoming residents, and parent/guardians, to complete intake paperwork.
- Meet with each resident to set and review goals in individually-designed Personal/Transitional Plans, focusing on education, employment, good tenancy, health, accessing mainstream benefits, and life skills.
- Meet with each resident once a week for 1 hour to review individually-designed Personal Plan goals. Support them with any setbacks, and coach them toward successful accomplishment of each goal.
- Provide transportation under exceptional circumstances only (i.e., initial orientation, appointments that mandate DTP presence, group activities, and urgent medical needs).
- Implement program rules and policies, including creating appropriate behavioral contracts when needed. Perform urinalysis tests for residents as required.
- Coordinate resident activities (independent living skills classes, community events, purchasing supplies) as time permits.
- Maintain contacts, satisfactory working relationships, and timely communications with representatives of area schools, colleges, GED programs, employers, financial aid counselors, parent/guardians, and other persons essential to resident success.
- Perform casita searches for prohibited items and monthly housing inspections, including fire safety. Communicate facility maintenance needs promptly.
- Attend clinical supervision meetings with Clinical Supervisor, Reflective with Program Director and weekly staff meetings.
- Conduct exit interviews with residents. Communicate resident changes to TSS staff.
- Maintain a clean driving record based on DreamTree Project insurance requirements.
- Participate in the on-call phone rotation schedule.
- Other duties as assigned.
Additional Compensation:
- $100 = 30 Days after start date
- $100 = 60 Days after start date
Job Type: Full-time
Pay: $19.00 - $24.00 per hour
