Job Overview :
As a Registered Physical Therapist (PT), you will provide physical therapy services to patients in their homes based on physician orders and care plans, under the guidance of the Director of Rehabilitation. Primary Responsibilities :
- Comply with established policies and procedures.
- Deliver physician-prescribed physical therapy.
- Work to improve or reduce patients' residual physical disabilities.
- Help patients achieve optimal levels of function within their capabilities.
- Collaborate with healthcare team members in patient care planning.
- Direct and supervise personnel as required.
- Obtain patient histories and conduct initial evaluations.
- Perform skilled procedures as prescribed by the physician.
- Engage with physicians regarding treatment alterations.
- Document patient progress for physician review.
- Instruct patients and their families on home programs and daily living activities.
- Participate in and possibly lead in-service programs.
- Engage in performance improvement initiatives.
- Attend scheduled patient care conferences.
- Prepare and maintain clinical and progress notes.
- Assist physicians with evaluating functional levels.
- Contribute to the development and refinement of care plans.
- Consult with families and agency personnel.
- Complete OASIS assessments and other required documentation according to defined timeframes.
- Accurately utilize ICD-9 codes.
- Stay current through ongoing participation in training and professional development.
- Fulfill additional duties as assigned by supervisors to support departmental effectiveness. Benefits :

- A valid PT license in good standing from the State of New York.
- A minimum of two (2) years of experience preferred.
- A graduate of a Physical Therapist program accredited by relevant governing bodies.
